What to expect: We'll discuss your goals, lifestyle, medical history and current eating patterns. This will help to identify key areas for attention, along with your strengths and challenges.
From here, together we'll develop the strategies that will help you to address your health concerns and move towards your goals.
Duration: Approximately 20-30 minutes
What to expect: Review appointments are really important to identify what's working well, what's not and to keep you moving towards your goals. Writing down any questions or concerns between appointments helps to make sure that you're getting the most from your appointments.
Review appointments may be available by phone.
Referrals: A referral from your GP or specialist is not required. If you have been given a referral, please bring it along to your appointment.
Medicare & Department of Veterans Affairs: Our Emprise is registered with both Medicare and the Department of Veteran's Affairs.
Chronic Disease Management (formerly EPC) appointments: If your GP has provided a Chronic Disease Management Plan, a Medicare rebate of $53.80 per consultation is available. Please bring this paperwork to your appointment as the rebate cannot be processed without it.
Bulk billing is offered for pension and concession card holders with a Chronic Disease Management plan: If no pension or concession cards are held, a gap fee is payable.
Private Health Funds: Dietetic services may be claimable through your private health care fund. Please call your fund for more information. HICAPS claiming is available on site.
Cancellation policy: Please provide at least one business days' notice to cancel or change an appointment to avoid a cancellation fee of 50% of the missed appointment fee. Being more than 10 minutes late will be considered a missed appointment and a cancellation fee will be payable.
Phone appointments are available from 9am to 5pm on Wednesdays and Thursdays.
Please note that these appointments are not eligible for the Medicare rebate, and may not be eligible for a rebate with some private health providers.
